I/ Staff Questionnaire

1. How well do you feel that training and development needs are prioritized for you and others in your area?

2. How comfortable are you with finding training or professional development when you have the need?

3. How do you feel about the availability of training and materials to develop you professionally?

4. What resources do you utilize when you have a training or professional development need?

5. What are some up-and-coming technologies or large projects that you will be addressing within the next 6 months?

6. How well prepared do you currently feel for those changes and projects?

7. Do you think you need additional training? If so, what kind of training?

8. Describe a situation that you could have handled in a more positive/professional way if you had additional training. (Could be a technical need or a professional development need)

9. Have you sought out training to help you with this situation in the future? How effective was the training?

10. When you have a training or professional development need, where do you go?

11. Do you have any other thoughts, comments or insights that would be useful to us in this needs assessment process?

II/ Supervisor Questionnaire

1. How do you see Company Professional Development’s involvement with your department currently? How would you like to see that relationship evolve?

2. What are some up-and-coming technologies or large projects that your area will be addressing with the next 6 months?

3. How do you quantify success on projects in your area (customer satisfaction, low amounts of down time…)?

4. If you have a project that is not successful, have you used training to address the issue? Was the training useful?

5. Describe any challenges that your area has had with training or professional development

6. Describe any successes that your area has had with training or professional development

7. When giving an annual employee performance evaluation, is individual professional development included?

8. If so, how do you prioritize their requests? How do you or the employee ensure that the professional development is completed?

9. Do you have any other thoughts, comments or insights that would be useful to us in this needs assessment process?

III/ Director Level Questionnaire

1. How do you see Company Professional Development’s involvement with your department currently? How would you like to see that relationship evolve?

2. What are some up-and-coming technologies or large projects that your area will be addressing within the next 6 – 12 months?

3. What is your current methodology for sending staff to training or professional development opportunities to address new technologies or large projects?

4. When you have sent staff to training, do you find that the training effective for staff working on these projects?

5. When giving an annual employee performance evaluation, is individual professional development included?

6. If so, how do you prioritize staff requests? How do you or the employees ensure that the professional development is completed?
