The self-evaluation process is designed so that you can evaluate your own performance and skills, set goals for the upcoming year, and highlight areas of concern. This will help you and your supervisor to have a frank and meaningful discussion.
A copy of this form should be retained by the employee’s supervisor, a copy should be given to the employee, and a copy should be sent to the HR Department for the permanent employee file.
1. What are your three most significant accomplishments in the past year?
2. What disappointments/failed expectations related to your work and employment at the Diocese did you experience in the past year?
3. Is there anything that gets in the way of your performing to best of your ability?
4. What are three areas where you think you need development?
5. What are the three best strengths that you bring to your position?
6. How would you rate your performance over the past year (please explain)?
7. What goals do you have for the next year?
a. How do you plan to achieve these goals?
b. What support do you need from your supervisor/others to reach your goals?
8. Is there anything that you would like to change about your position?
9. What do you need from your supervisor that you are not currently getting?
